What is sport and exercise medicine?

Sport medicine physicians care for active patients of all ages.  They diagnose and treat injuries to muscle, tendon, bone, nerve and ligaments that do not necessarily occur secondary to a sport.  They also diagnose and treat sport concussions.  Sport medicine physicians work with patients to prevent injuries.  They also help patients with arthritis become more active through exercise, injections and bracing.

Do I need a referral to see Dr. Van Boekel?

Yes, a referral from a physician is required for good communication with your family doctor and to ensure we have all information surrounding your injury to help treat you.  Dr. Van Boekel will see recent
sport concussions without a referral.

What do I need to bring to an appointment?

Shorts for leg or hip issues
Sleeveless top or tank top for arm or shoulder issues
Any braces you have for this injury
OHIP card/Health Card
List of current medications

Are there any special instructions for my first appointment?
Please arrive ten minutes early for your first appointment in order to fill out paperwork.

Is there accessible parking/entrance?
Dr. Van Boekel's sport medicine clinic is located at the far end of the lobby of the Stratford Rotary Complex.  This building has accessible parking and entrances.  There are also four parking spaces for Dr. Van Boekel's patients across from the front entrance to the Rotary Complex.

Where is the clinic located?
Dr. Van Boekel's sport medicine clinic is located in the Stratford Rotary Complex at 353 McCarthy Road in Stratford, Ontario.  When you turn into the parking lot the Rotary Complex is on the right side just after the Banquet Hall building. If you get to the Agriplex you have gone too far.

There is both accessible parking and parking spots designated for Dr. Van Boekel's patients directly across from the Arena doors to the Stratford Rotary Complex.  Once you enter the sliding doors you continue to the far end of the main floor lobby past the two ice pads.  There are two grey doors at the very end of the lobby and Dr.Van Boekel's name is outside of the doors.

Is there a cost/fee for the appointment with Dr. Van Boekel?
Your first visit and follow up visits are covered by OHIP as long as you have a valid health card.  Some procedures are not covered by OHIP but may be covered by your extended health care plan.  We accept debit and cash for payment for procedure costs.

Missed appointments
We require 24 hours notice for cancelling your appointment.  If you cancel less than 24 hours prior to your appointment then there is a rebooking fee if you would like to schedule another appointment.  If you provide your email address we are happy to send you an email reminder about your upcoming appointment.  Please call our office to cancel your appointment and do not reply to your email reminder.

Special instructions for concussion patients
Arrive 15 minutes early for your first appointment to complete medical information paperwork and a concussion questionnaire.
Please review the concussion handouts in the                            tab  prior to your first appointment.  They provide guidance for you prior to your first appointment.  Please arrive five to ten mintues early for follow up appointments to complete the concussion questionnaire.
